A bit like aircraft, intubating and extubating are the awkward and skill-requiring parts of a ventilator journey. Well, not so much the tube removal itself, but the choice of when to do it. When we ask our PICO questions, how often do we think about what we’re not asking … and when should we revise them? We’re never going to be satisfied with the search of direct randomised evidence in paediatrics - unless we’re working in the hallowed hills of 'paed oncology' of course - so how should we think about the weight of evidence to change things?
